Your rights and our responsibilities

This draft is designed for Little Kids Therapy and must be reviewed and adopted by the clinic before final publication. It will apply to Little Kids Therapy’s clinical records and operations to the extent required by applicable privacy laws.

01How we may use and share your information

We may use or share medical information to provide and coordinate treatment; bill and obtain payment; and conduct health care operations such as quality review, training, compliance, planning, and administration.

We may also share it with you or your authorized personal representative, and in other situations permitted or required by law, such as public health and safety, health oversight, workers’ compensation, law enforcement, judicial or administrative proceedings, and preventing a serious threat when legally permitted.

02When we need authorization

For uses or disclosures not described here or otherwise permitted by law, we will request written authorization. This includes selling medical information and certain marketing uses when authorization is required. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

03Your record rights

  • Inspect or obtain an electronic or paper copy of your medical information, subject to legal exceptions and permitted reasonable fees.
  • Request a correction if you believe information is incorrect or incomplete. We may deny the request in certain circumstances, but will explain why in writing.
  • Request a list of certain disclosures made during the period required by law.
  • Request restrictions on certain uses or disclosures. We are not always required to agree, except in specific situations provided by law.
  • Ask us to contact you in a reasonable alternative way or at an alternative location.
  • Obtain a paper or electronic copy of this notice.

04Minors and personal representatives

A parent, guardian, or other authorized person may exercise rights for the patient as allowed by law. Some situations allow a minor to control certain information or limit a representative’s access. We will verify authority and apply the appropriate rules before releasing information.

05Our responsibilities

The law requires us to protect the privacy and security of protected health information, notify you promptly if a breach may have compromised your information, provide this notice of our duties and practices, and follow the notice currently in effect.

We may change this notice and apply the new version to information already maintained and information received in the future. The current version will be available on our website and upon request.

06Questions or complaints

You may contact Little Kids Therapy with questions, to exercise a right, or if you believe your rights were violated. We will not retaliate for filing a complaint. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
